The municipal hospital in ČáslavThe hospital in Čáslav was founded in the year 1894 as a district hospital. The municipal hospital provides health care for citizens of three regions: Central Bohemia, region of Pardubice and region of Vysočina. There are almost 50 000 citizens living in these 3 regions. During the years 2000-2006 the hospital was completely reconstructed. Now it offers comfortable and functionally well designed environment for patients and staff. The hospital is fully designed to improve accessibility for patients with disabilities. In the hospital there are 195 beds. There is the department of internal medicine, department of gynaecology and obstetrics, department of surgery, department of paediatrics. Geriatrics is a part of the hospital, too. The citizens and physicians use also services of the very well equipped department of radiology, department of clinical biochemistry, department of haematology and blood transfusion. All departments use a modern medical information system allowing communication between a ward and outpatient services. The information system allows multimedia data transfers. It contains a managerial application instrumental for evaluation of economics, quality of health care and communication with health insurance companies. In the hospital there are 36 physicians working, 25 of them with attestation of the 2nd degree, and 112 nurses. In the year 2006 there were 6239 hospitalized patients and in twelve specialized outpatients’ departments 153 824 ambulatory examinations were carried out. According to the votes of twelve thousand respondents who participated in electronic voting in the frame of the national project Hospitals of the Czech Republic 2006, the Municipal hospital in Čáslav won 2nd place. The department of internal medicine is a part of health care network in the region of Central Bohemia. Staff, technical equipment and high level of services allow providing of a high quality care for patients. Complexity of care is ensured by an extensive outpatient department with specialists in the fields of cardiology, diabetology, gastroenterology, endocrinology, haematology and rheumatology. One part of the cardiologic outpatient department is the outpatient department of preventive cardiology, which is focused primarily on preventive examinations with regards on atherosclerotic cardiovascular diseases. Its task is to search for patients threatened by ischaemic heart disease, to provide extensive consulting services and to start preventive precautions. In addition to provided medical and hospital services the outpatient department manages a medical database for interdisciplinary research. Since 1998 the municipal hospital in Čáslav has been closely cooperating with the European Centre for Medical Informatics, Statistics and Epidemiology in the Institute of Computer Science AS CR, v.v.i. in Prague. Within the frame of this cooperation physicians of the department of internal medicine have participated in several projects. Since 2006 the hospital has become one of the organizations participating in the joint workplace, the Centre of Biomedical Informatics (CBI). Within the frame of this cooperation physicians of the department of internal medicine in the year 2003 managed to create a pilot model for registry of acute myocardial infarction, which allows acquiring complete data about occurrence of myocardial infarction in the hospital region, realistic data about risk factors, diagnostics and treatment. At the present time the registry is used in three former districts of CR. Results from the registry allow each working department to compare procedures and treatment with the Czech society of cardiology guidelines for care of patients with acute myocardial infarction and the results further allow comparing quality of provided care among particular hospitals. It also provides longitudinal data studies for e.g. trends in treatment procedures. In 2005 physicians of the department of internal medicine managed to create the data model of patients’ observations in secondary prevention of ischaemic heart disease. The pilot project was initiated on January 1st, 2006. Gathered information will lead to improvement of the quality of medical care in the region. In the department of internal medicine a substantive attention is paid to education within the frame of risk factors intervention of ischaemic heart disease. With the help of informational leaflets and data projection there are organised educational discussions for patients with ischaemic heart disease and obesity. Within the frame of hospitalization of patients with diabetes there are regular educational discussions leaded by a specialized nurse from a diabetological centre. The recondition centre for moving activities of obese people leaded by instructors educated in these problems was opened in the year 2006 within the cooperation of the physicians engaged in the Centre of Biomedical Informatics and of VŠTJ Medicina Praha.
